As accurate as the coordinates you provide. If you enter GPS coordinates with six decimal places, the QR code will point to a spot within roughly one meter. Using a street address relies on the map app's geocoding, which is generally accurate to the building level.
The QR code uses the standard geo: URI format, which lets the phone choose its default map app. On iPhones, that's typically Apple Maps. On Android, it's usually Google Maps. Users can also change their default map app in their phone settings.
The QR code itself can be scanned offline since the location data is embedded directly in the code. However, the map app will need some form of connectivity to load the map and provide navigation. If the user has offline maps downloaded, it can work without service.
